Dr. Fiaz Jaleel, MD, CLCP, is a board-certified physician specializing in Physical Medicine & Rehabilitation and Pain Medicine. After earning his medical degree from the University of the West Indies, Dr. Jaleel completed an Internal Medicine internship in Chicago, followed by a PM&R residency at Barnes-Jewish Hospital in St. Louis, affiliated with Washington University. He later founded Florida Pain Solutions in Altamonte Springs, where he focuses on acute and chronic pain, musculoskeletal disorders, and electrodiagnostic evaluations. Jordan Jorgensen comes to Exalt Life Care Plans with clinical experience after working day in and day out in treating clinics in Tampa, Florida. Jordan’s personable approach to every case ensures that patient care and client experience are the top priority.  Jordan holds a BA in Advertising and Public Relations from Florida Southern College, where she was also a collegiate athlete. She now shares her love of sports by coaching youth baseball. Jordan has an affinity for organization and spreadsheets and is happy to work in a place she loves every day.

As a healthcare compliance officer and operator in traditional healthcare and personal injury, Ms. Priest identified the clinical and operational need of streamed line future cost projection. Her passion for patient advocacy and efficient process development fueled the model developed for Exalt Life Care Plans providing patients and clients a service model delivering a new standard of excellence centrally focused on the needs of the injured supported by data driven standards of care.
